Divalproex Sodium
Nudival (Divalproex Sodium) is a stable co-ordination compound comprised of sodium valproate and valproic acid in a 1:1 molar relationship and formed during the partial neutralization of valproic acid with 0.5 equivalent of sodium hydroxide. Valproic acid is clear, colorless to pale yellow, slightly viscous liquid, and sparingly soluble in water, whereas sodium valproate is a white crystalline, hygroscopic, and highly soluble in water and alcohol. Divalproex sodium is fatty acid with anticonvulsant properties used in the treatment of epilepsy. It is used for the management and treatment of seizure disorders, mania, and prophylactic treatment of migraine headache. It has a role as an antimanic drug, an anticonvulsant and a GABA agent.

MODE OF ACTION:
The exact mechanisms by which divalproex sodium exerts its effects on epilepsy, migraine headaches, and bipolar disorder are unknown however several pathways exist which may contribute to the drug’s action.
Valproate is known to inhibit succinic semialdehyde dehydrogenase. This inhibition results in an increase in succinic semialdehyde which acts as an inhibitor of GABA transaminase ultimately reducing GABA metabolism and increasing GABAergic neurotransmission. As GABA is an inhibitory neurotransmitter, this increase results in increased inhibitory activity. A possible secondary contributor to cortical inhibition is a direct suppression of voltage gated sodium channel activity and indirect suppression through effects on GABA.

Dosage
Recommended Adult Dosage
Migraine Prophylaxis
Initial Dose: 250mg twice a day for 1 week
Optimal Dose: 750 to 1500mg/day
Bipolar Disorder (Manic Episodes)
Initial Dose: 25 mg/kg orally once daily
Optimal Dose: Not to exceed 60 mg/kg/day
Epilepsy
Initial Dose: 10-15mg/kg/day
Maintenance Dose: Increase by 5-10mg/kg/day every week
